The wish increases your level by one. For example, if you make this wish and are level 11, you will jump to level 12. If you don’t use it, this wish disappears after hitting level 80, the max. It might change if the level increases to 100 later on, then the wish will disappear at that level.
If you do use the wish and are still under the max level, it will disappear too, and you can only use it once.
The exception is if you complete the Guru’s house quests, Guru will eventually give you the wish back as “I want to grow more”. You still have to be below the max level, however.

Wish #1 – I want money!
It is an unlimited wish, and you can use it several times. You will get 500,000 Zeni on every use.
Wish # 2 I want a rare item!
You can only use it once, and you will get a Goodie radish (an accessory) as a reward.
Wish # 3 – I want to dress up!
You can use it two times. On 1st time, you will get a crystal battle suit and a gold battle suit the second time.
Wish #4 I want to get stronger!
It is an unlimited wish, and you can use it several times. You will get a soul echo device (6 of them), and a Soul diffusion mix dev (3) on every use.
You can use Soul Echo devices and soul diffusion mix to create super souls.
Wish #5 I want a new ultimate attack!
You can sue this wish two times, and you will get ultimate attacks for your created character. First, you will get “time skip/ Molotov (hit)”. While Ice Cannon (Eis Shenron) and Burning spin (Nuovo Shenron) on the second time.

Wish #6 I want a new super attack!
You can use it two times, and you will get super attacks for your creed character.
Flash fish crush(hit) on the first time.
While Ice claw (Eis Shenron), Ice field (Eis Shenron), Burst attack (Nuova Shenron), and Burst Reflection (Nuova Shenron) on the second time.

Wish #7 – I want more usable characters!
You can use this wish four times, and you will get more playable characters.
You will get Hit on the first time.
You will get Eis Shenron the second time.
You will get Nuova Shenron the third time.
And Omega Shenron for the fourth time.
Wish # 8 – I want a second chance at life!
It is an unlimited wish so that you can use it anytime. You can reassign your attribute points however you please with this wish. At level 80, there are 332 attribute points.
Wish # 9 I want to be drop-dead gorgeous!
It is an unlimited wish that will help you change the way you look. There are limitations like you can’t change your race or gender.
Wish #10 I want medals!
It is also an unlimited wish; you can use it as many times as you want. It will reward you with ten medals (like Zeni, you can use them to buy skills, items, etc.).
Wish #11 – I want to dress up more!
You can only use it once, and you will get a Turtle Hermit set (an accessory) as a reward.
Wish # 12 I want another new ultimate attack!
You can use this at one time. It will reward you with a minus energy power ball (Omega Shenron) and an ultimate attack for your created character.

You can only make this wish once, and you will get the following super attacks for your created character:

It is an unlimited wish, so you can make it as many times as you like. You will get 30(cups) of pudding as a reward, and you can use them to feed Majin Buu and his kids.
